About the Role
We are looking for a compassionate and driven Care Manager to support our members in navigating their healthcare journey. In this role, you will work closely with members, families, and healthcare providers to create personalized care plans, improve access to services, and ensure high-quality, cost-effective outcomes.
Coordinate Care & Advocate for Members
- Act as a key liaison between members, families/caregivers, and healthcare providers.
- Ensure timely access to appropriate healthcare services and community resources.
- Advocate for members to receive the care and support they need.
Provide Guidance & Support
- Educate members and caregivers on healthcare options, benefits, and available services.
- Offer psychosocial support and connect members to resources such as housing, employment, food assistance, education, and more.
- Guide members through complex healthcare systems with empathy and clarity.
Collaborate with Care Teams
- Work closely with providers, specialists, and interdisciplinary teams to coordinate care.
- Refer members for additional evaluation or services when appropriate.
- Support continuous improvement of care delivery through collaboration.
Education & Experience
- Bachelor’s degree required.
- 2–4 years of relevant experience in healthcare, care management, or a related field.
