Job Title: RN Care Manager – Complex Care Coordination
About the Role
We’re seeking a compassionate and experienced RN Care Manager to support complex and high-acuity members across the continuum of care. In this role, you will develop personalized care plans, coordinate services, advocate for members and families, and help improve health outcomes through high-quality, cost-effective care delivery.
Key Responsibilities
  • Assess complex member needs and develop individualized care plans/service plans
  • Coordinate care across providers, specialists, caregivers, and community resources
  • Monitor member progress, clinical status, and care outcomes; update plans as needed
  • Educate members and caregivers on healthcare options, benefits, disease management, and available services
  • Advocate for members to ensure access to person-centered, high-quality care
  • Collaborate with interdisciplinary teams to improve quality, compliance, and service delivery
  • Maintain accurate documentation in accordance with state, federal, and clinical guidelines
  • Conduct home or site visits as required
  • Support onboarding and mentorship of new clinical team members when applicable
Requirements
  • Graduate of an accredited School of Nursing or Bachelor’s degree with 4–6 years of related experience
  • Active RN or NP license in the applicable state (or Compact State Licensure)
  • Experience in care management, case management, long-term care, or managed care
  • Strong clinical assessment, care coordination, and communication skills
  • Ability to manage complex, high-acuity populations
Preferred Qualifications
  • Bachelor’s degree in Nursing (BSN)
  • Experience working in a Managed Care Organization (MCO)
  • Long-term care or population health management experience
  • Resource Utilization Group (RUG) certification (required within 90 days of hire for Superior)
